Payment
- Cash, Check, and Credit card (Visa and MasterCard, both debit and credit).
- Payment is due at the time of visit.
Insurance
- We accept most major forms of insurance.
- Unfortunately, we are not accepting – Medicaid, Humana Gold Choice HMO, PHCS, or Tricare as a primary insurance.
Non-Covered Services
- Please be aware that certain conditions and visits are not covered by health insurance. These include but are not limited to:
- Weight Loss assistance with Medifast, Phentermine, or other diet aids.
- Drug and Alcohol Detox.
- Laboratory blood draw fees (on some plans).
- Some procedures or bloodwork (ex: flu shots, other immunizations, PSA, screening EKG’s, Chest X-ray) are not covered benefits on some insurance plans.
- Cosmetic Procedures.
The patient is responsible for non-covered services even when it is bundled with a covered office visit.
Our office handles dozens of insurance plans and the carriers are constantly changing. While we will do our best to work with you regarding your individual plan, it is the patient’s responsibility to know what their insurance does and does not cover. |
